Payment Form Guidelines

1) This is for SAM/SAA members to register and pay for their students for SAM Graduation.  This form is intended for Teachers Use Only. 
2) Payment should be for all student participants. 
3) After you click on "Pay Now" you will need to enter the total cost for all students ($30 for each students, plus $10 for each additional level).
